Before proceeding for filling the form online the applicant must have the following essential documents:

  1. Scanned copy of residential certificate for permanent address from Gram Panchayat Prodhan/ Sabhapati, Panchayat Samity/ Member, Zilla Parisad Prodhan/ Local MLA/MP/ Commissioner/ Chairman of Municipality /Councilor/ Chairman of borough Committee of Municipal Corporation. (Less than 500 KB in JPEG format) .
  2. Scanned copy of Mark sheet of Madhyamik or its equivalent. (Less than 500 KB JPEG format)
  3. Scanned copy of Admit Card/Certificate of Madhyamik or its equivalent. (Less than 500 KB JPEG format)
  4. Scanned copy of Certificate of cast form appropariate authority .(Less than 500 KB JPEG format)
  5. Scanned Photograph. (Less than 200 KB JPEG format)
  6. Scanned Signature. (Less than 200 KB JPEG format)

It is suggested to scan documents form your originals. There is no guarantee of Govt. service for the candidates after satisfactory completion of the Course. No correspondence will be entertrained in this issue.
